Empyema is a serious health problem that can affect individuals of all ages. Although there has been some change in treatment modalities with the advancement of technology, there is still no standard approach to treatment. The most common cause of parapneumonic empyema is parapneumonic or postpneumonic effusions. Inappropriate diagnosis and intervention can result in chronic parapneumonic empyema. This can lead to pleural thickening, which can further reduce lung and thorax volume and impair respiratory function. In more advanced cases, more invasive surgical procedures may be needed, such as thoracotomy and decortication. Alternatively, myoplasty or thoracomyplasty may be employed. As the main principle in empyema treatment is to match the stage, early diagnosis and intervention are crucial. Consequently, the primary objective is to administer appropriate antibiotics, perform appropriate drainage, and, in cases of loculation, implement early debridement (fibrinolytic therapy, VATS) in order to reduce the necessity for thoracotomy.

3Dr. Atalay Şahin was born in Malatya in 1960. He graduated from Erciyes University Faculty of Medicine in 1989. Between 1994–1999, he completed a thoracic Surgery specialisation at Erciyes University, Faculty of Medicine, Thoracic and Cardiovascular Surgery. From 1999 to 2000 he worked as a chief physician in Yozgat State Hospital. Between 2000 and 2011, he worked at Kayseri Regional Hospital and Kayseri General Hospital. He speaks English and intermediate German. During his education and specialisation periods, he did volunteer works in Britain and Germany. He worked on temporary assignments in field hospitals in Sudan and Saudi Arabia. He worked as an assistant professor at Dicle University Faculty of Medicine, Department of Thoracic Surgery, between 2011-2014, and as a specialist at Diyarbakır Training and Research Hospital between 2014-2017, and became an associate professor here in 2015. In 2017, he started working as an associate professor at Harran University Faculty of Medicine, Department of Thoracic Surgery, and I had to leave here in 2021.In 2021, I was appointed as a professor at Adıyaman University Faculty of Medicine, Department of Thoracic Surgery and he has been still working.Asoc.professor.

3Menduh ORUÇ completed his undergraduate education at Dicle University Faculty of Medicine between 1996-2001 and his thoracic surgery specialization at Dicle University Faculty of Medicine between 2004-2009. He started his academic career as an assistant professor at Dicle University Faculty of Medicine in 2013. He received the title of associate professor in 2019 and still continues as an academic staff member at the same university. Dr.Menduh ORUÇ is a member of the Turkish Thoracic Surgery Association, Turkish Thoracic Association, Turkish Respiratory Research Association, Turkish Lung Cancers Association, Turkish Pulmonary Hydatidology Association and EACTS. Skills:He plays chess well, speaks Turkish at a very good level, as well as English at an intermediate level.Apart from these, he focuses on agriculture and plants as a hobby.
