Acute exacerbations are sudden, severe worsening of respiratory symptoms, including dyspnea and cough, accompanied by rapid decline in lung function. Acute exacerbations are associated with high morbidity and mortality rates and may require hospitalization and intensive care management. Increased pulmonary vascular resistance leading to elevated pulmonary artery pressures and right heart strain. Pulmonary hypertension is a common complication of IPF and is associated with worse prognosis and reduced survival. Increased susceptibility to respiratory infections, including bacterial pneumonia, viral respiratory tract infections, and fungal infections, due to impaired lung function and compromised immune response may develop.
Gastroesophageal Reflux Disease (GERD) is a common comorbidity in idiopathic pulmonary fibrosis patients, with reflux of gastric contents into the esophagus potentially exacerbating pulmonary fibrosis through microaspiration and inflammation.
Increased risk of venous thromboembolism, including pulmonary embolism, in idiopathic pulmonary fibrosis patients may emerge due to impaired lung perfusion, immobility, and hypercoagulability. Reduced bone mineral density and increased risk of fractures due to chronic corticosteroid use, physical inactivity, and systemic inflammation. High prevalence of depression, anxiety, and psychological distress in idiopathic pulmonary fibrosis patients, attributed to chronic illness, physical limitations, functional decline, and uncertainty about prognosis.
Acute exacerbations are a significant predictor of poor prognosis, with increased risk of disease progression and mortality. Advanced age is a negative prognostic factor in idiopathic pulmonary fibrosis, with older patients generally experiencing more rapid disease progression and shorter survival compared to younger individuals. Presence of comorbid conditions, such as cardiovascular disease, diabetes mellitus, and chronic kidney disease, is associated with increased mortality risk in idiopathic pulmonary fibrosis patients. Various prognostic scoring systems, such as the GAP index (gender, age, physiology) and the ILD-GAP index (interstitial lung disease-GAP index), integrate clinical, radiological, and physiological parameters to stratify idiopathic pulmonary patients into risk categories and predict survival outcomes.

In ancient medicine, many scientists have mentioned that the human body has its own mechanisms to defend against diseases and achieve either a cure or the symptomatic response. In the 5th century B.C., the ancient Greeks noted that patients who survived the plague disease were somehow immune to it. Ibn Sina, in his book ’The Canon of Medicine,’ describes the basics of our immune system with general concepts. Ibn Sina had mentioned that the human body has its own defense power and called it ’Quatu-almudafat’ in the Arabic language. Not only the contagious behavior of some diseases (person to person) and the distribution nature of them (through water or solids), but also, he mentioned or believed that these contagious diseases need 40 days to be under control and limit the spread of infections. However, the term ’immunology’ was much later coined by the Russian biologist Ilya Ilyich Mechnikov, who received the Nobel Prize in 1908 with Paul Ehrlich for their study and work on immunity. There have been rapid advances in immunology, as in every field of science in the 20th and 21st centuries. Developments in biology, microbiology, drug production and advances in many fields have led to the recognition of cell antigenicity, the definition of foreign antigen and antibody relationships, and finally the treatments we call Immunotherapy, in which humoral and cellular components have been developed today, as well as treatments with antibodies alone or antibody drugs, antibody toxins, antibody radioactive congucation. In addition cellular therapy mediated by T cells and NK cells (such as CAR-T cell, CAR-NK, LAK) have been developed.
Today and in the near future, immune therapies seem to play a role in many diseases other than cancer. Immune therapies alone or with drug/toxin conjugates are promising in cancer treatments and have led and will lead to very long-term survival in some cancers.
This book, which is the book on immune therapies in cancer, has been prepared by doctors who are experts in the field of immune therapies in some hematological and non-hematological cancers. Information on the subject is the responsibility of the author. We hope that this book, in which you will read many innovations, will be useful.

Acute otitis media is a sudden onset infection of the middle ear and is usually more common in children. This infection is caused by bacterial or viral agents that can occur after upper respiratory tract infections. Symptoms include severe ear pain, fever, hearing loss and sometimes discharge from the ear. Acute otitis media can cause restlessness, sleep problems and feeding difficulties in children. Antibiotics are commonly used to treat it, but in some cases the infection may resolve on its own. Early diagnosis and treatment is important to prevent complications.

Approximately 10-20% of idiopathic pulmonary fibrosis cases have a familial component, suggesting a strong genetic influence. While most cases are sporadic, familial cases provide critical insights into genetic predispositions and mechanisms. TERT and TERC mutations which encode components of telomerase, are among the most common in familial idiopathic pulmonary fibrosis. They lead to shortened telomeres and premature cellular aging. SFTPC and SFTPA2 mutations in genes encoding surfactant proteins can disrupt normal lung function and homeostasis, leading to increased fibrosis. MUC5B is a common polymorphism in the promoter region of the MUC5B gene is strongly associated with both familial and sporadicidiopathic pulmonary fibrosis. This variant increases the expression of mucin, which may contribute to aberrant wound healing and fibrosis.

This chapter deals with marriage processes and the emotional and psychological states experienced by couples during these processes. Love and affection form the basis of relationships, and the dating period is the process of couples getting to know each other and taking the relationship to marriage. According to Alan Lee’s “Colors of Love” theory, there are three basic forms of love: passionate love (eros), playful love (ludus) and friendly love. These three styles of love form the basis of other love styles. Additionally, attachment styles also affect relationship dynamics; such as secure attachment, preoccupied attachment, dismissive attachment and fearful attachment. The engagement process is a stage where the marriage proposal is accepted and families are involved, where couples have the opportunity to get to know each other better. The engagement process is a larger and more crowded event and is a period in which families and relatives recognize the relationship. For a healthy relationship and marriage, it is important for couples to know each other and achieve emotional and logical harmony. Secure attachment and empathy in the premarital period strengthens the relationship. When couples show love, respect and value to each other, their relationships become healthier and more permanent. This document aims to help couples understand the challenges they may face on the road to marriage and how they can overcome these challenges.

The human brain is one of the important organs in the human body. It is the most complex of all organs. The brain is an organ composed of billions of nerve cells. It has parts of the brain that have important functions1. It is important for researchers to have details about brain anatomy and function that contribute to brain diseases and brain damage. The brain receives information about the body surface and its changes inside the body through sensory nerve receptors. After the interpretation of this information, it sends messages for the functioning of the muscles and glands via motoric. The brain is the center of all conscious action. It has lobes with different functions. The brain manages all the functions of the body. It interprets the information coming to the brain from the inner body of the human and also from the environment in which he lives. As a result of the brain’s control and interpretation of information; the individual realizes himself/herself and perceives the world. The brain receives information about both the changes in the body surface and the events occurring in the body through sensory nerve receptors. The brain receives information through the senses. The brain processes and interprets the information it receives. The brain, cerebellum, brainstem, and spinal cord are parts of the central nervous system. 12 pairs of cranial nerves, 31 pairs of spinal nerves, neural networks and ganglia are the components of the peripheral. Whole brain; consists of the brain, cerebellum, and brain stem. The whole brain is called the Encephalon. The largest and most recognizable part of the entire brain is the cerebrum. On the outside of the brain is the cortex cerebri, which is the gray matter formed by nerve cells. On the inside, it consists of white matter formed by axons, which are nerve cell extensions. The cerebrum is divided into two hemispheres, left and right. Brain through some grooves and slits; It is divided into the frontal, temporal, parietal, and occipital lobes. Brain; It assumes roles that process thinking, learning, memory, language, emotion, movement and perception 2-4. As a result, in the most important organ, the brain; In order to achieve improvement, it is important to stay away from both the substance and the information and visuals that remind the use of the substance.
